When my coworkers and I first took up the task of writing a textbook of ophthalmology that was aimed at medical students but would also be suitable for interns and ophthalmology residents, we did not know exactly what wewere getting ourselves into.

The next four yearswere devoted to intensive study of this subject.We did not merely intend to design a book according to the maxims “understand it in medical school,” “learn it for the examination,” and “use it during your internship.”

Our broader goal was to give students a textbook that would kindle their interest and indeed their enthusiasm for a “small” specialty like ophthalmology and thatwould sustain this enthusiasm all the way through a successful examination.

In an age in which teaching is undergoing evaluation, we felt this was particularly important. In pursuing this admittedly ambitious goal, we were able to draw upon many years of teaching experience.

There are encyclopedic reference books available in many sub-specialty areas of eye care, including pediatric ophthalmology. These large texts are most valuable when a clinician needs to quickly find a differential diagnosis, a list of treatment options, or the findings to look for when a particular syndrome is suspected.

With Pediatric Ophthalmology: Current Thought and a Practical Guide, we have not attempted to match the breadth of those exhaustive reference texts. Instead, we bring to the pediatric-oriented ophthalmologists a book they will want to read cover-tocover.

We strived for enough depth and perspective in each chapter so that the book could be considered core reading for trainees and practitioners alike.

When I first met with Marion Philipp, Senior Editor at Springer, to discuss this project, I told her that this book would be the most well-read book in the pediatric ophthalmology field because each chapter would be written by a respected thoughtleader who could give a concise overview of the most current thought and practice recommendations for that subject.

In the preface to the second edition of this book we stressed that we had written the text in the hope of helping those trainee eye surgeons who were preparing to take their basic professional examinations.

We assumed that the optics they had learned at school had long since been forgotten, and we therefore set out to explain the subject from first principles.

Our aim was to keep the text logical and simple and we used diagrams liberally to complement the written word. These diagrams themselves were simplified so that they could be both easily memorised and reproduced – even under the stress of examination conditions.

Throughout the book we emphasised the clinical relevance of each topic and we hoped that this approach would allow readers to understand better the optical problems experienced by many of their patients...

Basic Ophthalmology is designed to help the user obtain an appropriate ocular history and learn the examination techniques for a complete eye evaluation.

From the history and clinical findings the reader should be able to diagnose and manage or refer common ocular disorders. The history of this textbook began in 1975 with the publication by the American Academy of Ophthalmology of a study guide in outline form for medical students.

The book’s developers identified seven common problem areas in ophthalmology and developed study objectives. Each subsequent edition was changed based on suggestions from users.

The fifth edition, which was developed by the joint committee of the Amer­ican Academy of Ophthalmology and the Association of University Professors of Ophthalmology, abandoned the outline form for chapters with expository text.

Do you read imperfectly?

Can you observe then that when you look at the first word, or the first letter, of a sentence you do not see best where you are looking; that you see other words, or other letters, just as well as or better than the one you are looking at?

Do you observe also that the harder you try to see the worse you see?

Now close your eyes and rest them, remembering some color, like black or white, that you can remember perfectly. Keep them closed until they feel rested, or until the feeling of strain has been completely relieved. Now open them and look at the first word or letter of a sentence for a fraction of a second. If you have been able to relax, partially or completely, you will have a flash of improved or clear vision, and the area seen best will be smaller.

After opening the eyes for this fraction of a second, close them again quickly, still remembering the color, and keep them closed until they again feel rested. Then again open them for a fraction of a second. Continue this alternate resting of the eyes and flashing of the letters for a time, and you may soon find that you can keep your eyes open longer than a fraction of a second without losing the improved vision.

If your trouble is with distant instead of near vision, use the same method with distant letters.

In this way you can demonstrate for yourself the fundamental principle of the cure of imperfect sight by treatment without glasses.

If you fail, ask someone with perfect sight to help you.

Микроскопия живого глаза, или биомикроскопия, является чрезвычайно тонким и довольно точным методом исследования, который открывает широкие возможности изучения физиологии и патологии органа зрения.

При помощи биомикроскопии возможно исследование нормальных тканевых структур функционирующего глаза, наблюдение мельчайших изменений в нем.

Метод биомикроскопии необходим для постановки раннего диагноза целого ряда глазных заболеваний и проведения дифференциальной диагностики.

Микроскопия живого глаза позволяет наблюдать динамику патологического процесса и своевременно ориентироваться в выборе метода лечения, а иногда в выборе вида оперативного вмешательства с последующей оценкой результатов произведенной операции. Пользуясь биомикроскопией, можно уже в ранние сроки наблюдения за больным судить о прогнозе заболевания.

Данная монография будет способствовать более быстрому повышению квалификации офтальмологов нашей страны, повышению качества их диагностической, лечебной и профилактической работы.
